2024-2027 My Anesthesia Choice-HF: Implementing an Evidence-Based Approach to Increase Shared Decision Making about Anesthesia Options for Hip Fracture Surgery

Hip fractures are common; there are about 250,000 hip fractures/year in US and about 1.6 million/year worldwide. Spinal and general anesthesia are both reasonable options for patients undergoing surgery for hip fracture; there is strong evidence of similar patient-centered outcomeswith each across a range of endpoints. Many patients and families have questions and preferences about anesthesia options for hip fracture surgery; they often want to be involved in anesthesia discussions.
The My Anesthesia Choice project was created by researchers at the University of Pennsylvania (led by Dr. Mark Neuman) and Washington University in St. Louis (led by Dr. Mary Politi) to create tools and processes to help patients and their doctors make better decisions about anesthesia for hip fracture surgery.
Aim 1 will conduct qualitative interviews with clinicians and patients/caregivers to refine a conversation aid about anesthesia choices for hip fracture surgery, Aim 2 will engage in strategies to support implementation success and sustainability, and Aim 3 will conduct a stepped wedge cluster randomize trial in 6 sites to evaluate implementation, randomizing sites to the timing of when they start using the tool/process.
